We are seeking a skilled Patient Care Associate to join our team at Southwell, Inc.

About the Role:

The Patient Care Associate will play a vital role in providing patient care under the direction of a Registered Nurse or Licensed Practical Nurse. This includes assisting with daily tasks, maintaining cleanliness, and ensuring patient comfort and safety.

Key Responsibilities:

  • Perform blood sugar testing and report results to the nurse
  • Maintain unit area cleanliness
  • Follow proper processes for documentation and supply management
  • Conduct hourly rounding to meet patient needs
  • Document and communicate patient needs to nursing staff
  • Maintain patient safety through implementation of nursing safety interventions
  • Perform vital signs and document in EMR
  • Assist patients with ADLs as needed
  • Change patient bed linens
  • Record I&O in EMR
  • Transport patients as needed
  • Demonstrate discretion when handling confidential information
  • Make necessary copies of charts during patient transfer or discharge

Requirements:

  • High School Diploma or GED
  • Current Certified Nurse Aide certification accepted in lieu of education requirement
  • Successful completion of a Certified Nursing Assistant, PCT, or Medical Assistant program preferred

Benefits:

We offer competitive compensation and benefits, including an estimated salary range of $45,000 - $60,000 per year, depending on experience.
