Key Responsibilities:
- Provide ongoing support and expertise through comprehensive assessment, care planning, and plan implementation.
- Ensure patient preferences are respected while delivering care.
- Serve as a patient advocate through resource utilization, discharge planning, and addressing holistic patient needs.
Requirements:
- Must have BLS certification from the American Heart Association.
- 2 years of RN Case Management experience, with a focus on long-term acute care settings.
- Washington RN license.
- 6 hours of Suicide Prevention Training completed through the WA DOH website.
Work Schedule:
Monday - Friday, 8a-430p.
