Job Description

We are seeking a skilled Healthcare Social Services Coordinator to join our palliative care team. The successful candidate will be responsible for assessing the emotional and social needs of patients facing chronic illnesses, facilitating plans with patients and families to promote effective care, and maintaining a current knowledge base of available community agencies and resources.

Key Responsibilities:
  • Conduct thorough assessments of patients' emotional and social needs
  • Facilitate plans with patients and families to promote effective care
  • Maintain a current knowledge base of available community agencies and resources
  • Provide direct clinical therapeutic services to patients as part of an overall comprehensive patient care plan

The ideal candidate will possess a Master's degree in Social Work from an accredited program, licensure as a Social Worker in the State of South Dakota, and experience working in a healthcare setting.

Requirements

To be considered for this role, candidates must have:

  • A Master's degree in Social Work from an accredited program
  • Licensure as a Social Worker in the State of South Dakota
  • Experience working in a healthcare setting

Additionally, qualified candidates will have a Qualified Mental Health Professional Credential (in South Dakota) and multi-state licensure.
