About the Role

About You

Responsibilities
As an LPN/LVN at Aveanna, your primary responsibilities will include:
  • • Providing and documenting skilled nursing care to patients in accordance with their individualized care plans and physician orders.
    • Conducting thorough assessments of patients' physical and emotional needs, identifying areas for improvement and implementing strategies to enhance their quality of life.
    • Developing and maintaining effective relationships with patients, families, and healthcare teams to ensure seamless care coordination and communication.
    • Participating in interdisciplinary care planning and meetings to ensure that patients receive comprehensive and coordinated care.
    • Maintaining accurate and up-to-date records of patient interactions, progress, and outcomes, adhering to regulatory requirements and organizational standards.
    • Collaborating with other healthcare professionals to ensure continuity of care and optimal patient outcomes.

Requirements
To be considered for this exciting opportunity, you must possess the following qualifications:
  • • Graduate of an accredited school of nursing with a current, unrestricted state license as a Licensed Nurse in the state of practice.
    • Current CPR certification and ability to meet organization-specific competency requirements.
    • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
