About the Role

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at CenterWell Home Health. As a Home Health LPN, you will play a vital role in providing high-quality patient care and support to individuals in their homes.

Key Responsibilities
  • Provide professional nursing services under the direction of a RN, adhering to state and federal regulations, and CenterWell Home Health policies.
  • Develop and implement individualized care plans to address patients' medical, rehabilitative, and preventative needs.
  • Maintain accurate and detailed records of patient interactions, progress, and outcomes.
  • Collaborate with healthcare teams to ensure seamless care coordination and discharge planning.
  • Participate in ongoing education and training to enhance clinical skills and knowledge.
Requirements
  • Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
  • Current nursing license in the practicing state.
  • Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice care.
