Salary Information

The estimated annual salary for this role ranges from $88,000 to $173,200 in New Jersey. This compensation reflects a comprehensive benefits package, including performance incentives, recognition programs, equity stock purchase opportunities, and 401(k) contributions.

About the Role

We are seeking highly skilled Nurse Practitioners to join our Senior Community Care team. As a key member of our interdisciplinary team, you will be responsible for providing high-quality care to our most vulnerable patients in their homes, nursing homes, assisted living facilities, or senior housing communities.

Main Responsibilities:
  • Conduct thorough assessments to identify patient needs and develop effective care plans.
  • Manage acute medical situations or exacerbations of chronic conditions, including testing, diagnostics, and interventions.
  • Support patients during short-term sub-acute rehabilitation episodes.
  • Ensure accurate and complete documentation, including ICD-10 conditions.
  • Collaborate with the care team to deliver coordinated care.
Required Qualifications:
  • A Master of Science in Nursing degree from an accredited institution.
  • An active, unrestricted license to practice as a Registered Nurse in New Jersey (or ability to obtain by start date).
  • National certification as a Nurse Practitioner through AANP or ANCC, with certification in one of the following areas: Family Nurse Practitioner, Adult Nurse Practitioner, Gerontology Nurse Practitioner, or Adult-Gerontology Acute Care Nurse Practitioner.
  • A current DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations.
  • Ability to secure a collaborative practice agreement, if applicable in your state.
  • Capacity to meet the physical demands of the job, including lifting 30 pounds and navigating varied dwelling conditions.
Preferred Qualifications:
  • One or more years of clinical experience, preferably in long-term care settings.
  • Familiarity with meeting the complex medical needs of patients with behavioral, social, and functional challenges.
  • Understanding of geriatrics and chronic illness management.
  • Experience with advanced illness and end-of-life discussions.
  • Proficiency in electronic medical records and documentation.
