About the Role
We are seeking a highly skilled and compassionate Care Manager II to join our team. As a key member of our care management team, you will be responsible for assessing and planning for the well-being of our clients with complex medical and psychosocial needs. Your expertise will be instrumental in ensuring the provision of quality care and coordinating essential services.

Responsibilities

  • Comprehensive Assessments: Conduct thorough assessments of clients, including psychosocial, physical, and mental health, environmental, and spiritual needs.
  • Community Living Plans: Develop and initiate client-centered, comprehensive, and consistent Community Living Plans based on assessment information.
  • Service Coordination: Identify, arrange for, and monitor community services, including Medicare, Medi-Cal, and other entitlement programs.
  • Care Management Relationship: Establish and maintain a care management relationship with clients and their informal support network, offering respect, dignity, and support.
  • Crisis Intervention and Advocacy: Provide crisis intervention, advocacy, problem-solving, and therapeutic interventions as needed.
  • Documentation and Communication: Document case management activity, maintain required paperwork, and communicate effectively with community providers.
  • Continuous Quality Improvement: Participate in ongoing efforts towards Continuous Quality Improvement, attending team and program meetings, and contributing to open lines of communication.
  • Education and Training: Maintain up-to-date knowledge and skills through continuing education classes, in-service training, and other relevant resources.

Requirements

  • Education: Master's in Social Work or related field required.
  • Experience: One year working with disabled adults and/or older adults required, with experience in case management, gerontology, and community health care delivery systems highly desired.
  • Skills: Exceptional communication and presentation skills, detail-oriented, and ability to prioritize multiple tasks.

We offer a competitive salary range of $75,000-$86,000, as well as opportunities for growth and professional development in a dynamic and supportive environment.
