Key Responsibilities:

  • Maintain accurate and up-to-date case records in accordance with licensing and Medicaid standards.
  • Provide crisis intervention and case management services to assigned caseload.
  • Develop and implement Individualized Service Plans in collaboration with clients and multidisciplinary teams.
  • Conduct individual, family, and group counseling and therapy sessions, tailored to meet the unique needs of each client.
  • Document all treatment activities, including phone calls and sessions.
  • Collaborate with affiliated case professionals, including school, social services, and other mental health providers, to ensure comprehensive support.
  • Provide formal case updates and presentations to professionals in various settings.
