Job Overview

We are seeking a highly skilled and compassionate Psych Nurse Practitioner to join our team at Horizon Health Services. This full-time position offers the opportunity to make a meaningful difference in the lives of others while working in a supportive and collaborative environment.

Responsibilities:

  • Conduct initial psychiatric examinations for individuals admitted to mental health programs and provide follow-up medication management services.
  • Participate in clinical case conferences to provide consultation and guidance on difficult cases, ensuring effective communication with counseling staff regarding consumer progress and outstanding issues.
  • Prescribe psychotropic and/or addiction medication to alleviate symptoms, reviewing incoming physical exams to ensure accuracy and relevance.
  • Communicate referrals for physical exams with primary care physicians and document all services provided in the case record.
  • Maintain confidentiality and handle sensitive information with utmost care.
  • Review and sign treatment plans as needed, utilizing electronic health records effectively.

Requirements:

  • Master's Degree in a Nurse Practitioner Program and National Board Certification in specialty area required.
  • Current license to practice as a Nurse Practitioner in New York State and Registered Professional Nurse required.
  • Minimum 3 years' experience with individuals having mental health needs.

Physical Demands:

  • Stationary posture with frequent standing/sitting alternation.
  • Mobility between nearby worksites and within facilities.
  • Mobility requiring repetitive wrist, hand, and finger movements.
  • Lifting objects up to 20 pounds.
