About the Role

We are seeking a dedicated and compassionate Certified Nursing Professional to join our team as a Patient Care Specialist. In this role, you will have the opportunity to make a meaningful impact on the lives of our patients and their families.

Job Summary

The Patient Care Specialist will work under the direct supervision of a Registered Nurse to administer patient care and support the nursing process in providing individualized care to patients. This includes assisting patients in adapting to the disease process and achieving their highest level of wellness.

This is a Per Diem As Needed position, offering flexibility and autonomy in your work schedule. Our ideal candidate will possess excellent communication skills, both verbal and written, and be able to effectively communicate with patients, families, and healthcare teams.

Responsibilities
  • Maintains consistency with Administrative and Departmental policies through appropriate behavior, dress, attitude, attendance, confidentiality, professionalism, and reliability.
  • Ensures professional discretion and confidentiality of hospital-related information concerning patients, physicians, associates, and visitors.
  • Complies with conditions of employment as stated in the associate handbook and Administrative and Departmental policies.
  • Assists the RN in the nursing process to provide information for development of the plan of care.
  • Prompts reports identified changes in the patient's condition.
  • Reviews and utilizes diagnostic data, medical orders, and nursing assessment in order to provide continuity of care in accordance with the plan of care.
  • Demonstrates an ability to respond appropriately and/or report abnormal clinical/laboratory findings or results.
  • Sets priorities for care of patients based on acuity and/or patient/family preference.
  • Accurately reports and documents the patient's symptoms, responses, and status to reflect the nursing care plan developed by the RN.
Requirements

To be successful in this role, you will need:

  • A high school diploma or equivalent.
  • Basic computer knowledge.
  • Successful completion of LVN/LPN training.
  • Ability to communicate effectively, both verbally and written.
  • Prefer 6 months of clinical practice in an acute care environment.
Benefits
