We are seeking a skilled and compassionate Nurse Practitioner to join our team at Matrix Medical Network. As a Home Risk Assessment Nurse Specialist, you will play a vital role in improving the quality of care for older adults and other at-risk individuals.

Salary and BenefitsThe salary for this position is estimated to be around $90 per completed assessment. In addition to competitive pay, we offer a mileage reimbursement plan and flexible scheduling options. As a PRN/Part Time provider, you will have the opportunity to work independently while still enjoying the benefits of being part of a larger organization.
About the PositionAs a Home Risk Assessment Nurse Specialist, your primary responsibility will be to conduct comprehensive health assessments in the homes of our patients. This may involve reviewing medical history, observing current health status, and providing education and support to help patients manage their health. You will work collaboratively with physicians, case managers, and other healthcare professionals to ensure seamless care coordination.
RequirementsTo be considered for this position, you must have a Master's Degree in Nursing or a related field, as well as current RN and NP licensure in the state of practice. Board certification by the AANP, ANCC, or AACCN is also required. Additionally, you should have a minimum of 1 year experience as a Nurse Practitioner and strong computer skills. Excellent verbal and written communication skills are essential for success in this role.
Travel RequirementsThis position requires the ability to travel to meet patients in their homes. You will need a valid state driver's license and proof of adequate automobile insurance coverage. Travel will be necessary to visit patients in various locations, including Centreville, MS.
