We are seeking a highly skilled and dedicated Clinical Care Coordinator to join our team at Weill Cornell Medicine.

About the Role:

This is an exciting opportunity for a Registered Nurse to work in a dynamic and collaborative environment, providing high-quality patient care and supporting the clinical team.

Key Responsibilities:

  • Coordinate and organize the clinical care of patients within the practice, ensuring seamless delivery of services and exceptional patient outcomes.
  • Triage patient inquiries and communicate test results in a timely and compassionate manner.
  • Monitor medical supplies, ordering and receiving inventory as needed, and maintain accurate records of stock levels.
  • Develop and implement comprehensive plans of care based on individual patient needs, collaborating with healthcare professionals to achieve optimal outcomes.
  • Evaluate patient status in emergency situations, administering medications and coordinating appropriate care.
  • Schedule tests and procedures, educate patients and family members regarding treatment and therapies, and record diagnoses, treatment plans, and progress notes.
  • Assist with cost control and maintain accurate records of patient information.

Requirements:

  • Current NY State Registered Nurse License & Registration.
  • BLS Certification (issued by the American Heart Association).
  • Infection Control Certificate.
  • Associate's Degree in Nursing; Bachelor's Degree preferred.
  • Up to 2 years of post-degree course work and/or an internship and less than one year of previous experience may be considered.
