Nurse Practitioner

**Job Summary:**
We are seeking a skilled Clinical Care Coordinator to join our team at TulaRay. As a Clinical Care Coordinator, you will be responsible for providing high-quality nursing care to our residents. This includes assessing residents' medical needs, developing and implementing individualized care plans, and coordinating with healthcare practitioners to ensure residents receive the best possible care.
**Responsibilities:**
* Provide direct nursing care to residents, including administering medications, performing treatments, and monitoring vital signs
* Develop and implement individualized care plans to meet residents' unique needs
* Coordinate with healthcare practitioners to ensure residents receive timely and effective care
* Communicate effectively with residents, families, and healthcare teams to ensure seamless care coordination
* Maintain accurate and up-to-date medical records
**Requirements:**
* Possession of a current license to practice as a registered nurse issued by the Pennsylvania State Board of Nursing
* Ability to enter health data into electronic medical records
* Proficient computer skills, including Microsoft Office products
