Our integrated model of care delivers comprehensive services to adults residing in skilled nursing and assisted living facilities through a network of healthcare providers, including physicians, nurse practitioners, physician assistants, medical assistants, and support staff.
Key Responsibilities:- Provide primary-care services in accordance with Eventus WholeHealth site-specific protocols and consistent with the standard of care for family medicine, geriatric medicine, and internal medicine as it pertains to long-term care facility work.
- Review and sign MARs as requested by facilities, sign all orders specific for a supervising physician while at the facility or within 24 hours if faxed or emailed.
- Manage all scheduled medication prescriptions specific to your facility's patients and specialty, and as per Eventus WholeHealth's scheduled medication Rx policy.
- Provide facility with adequate time each visit to allow for paperwork and administrative duties.
- Work with facility to manage systems, attend care plan meetings, and QA/PI meetings as requested by facility and/or supervisor.
- Submit copies of documentation and billing records to Eventus WholeHealth according to the billing schedules.
- Establish and maintain open and positive communications with facility staff and administration, providing verbal and/or written instruction or feedback regarding medications and other pertinent caregiver information.
- Meet with Administrator or designated facility care coordinator equivalent prior to the start of your patient care visits each day and prior to leaving the facility each day.
- Be knowledgeable of and adhere to Eventus WholeHealth standards, policies, and procedures, as well as all legal and regulatory agencies' rules and guidelines and professional ethical standards.
- Maintain patient confidentiality at all times, including appropriate use of cell phones, emails, text messaging, patient charts, and EMR.
- Licensed MD or DO.
- 3 years in Long Term Care/Geriatric Care.
- AMDA Certified a plus but not mandatory.
- Strong leadership qualities.
- Strong organizational skills.
- Excellent written and verbal communications skills.
