Job Summary:

We are seeking a compassionate and experienced Registered Nurse (RN) to provide medical care and support to patients in their homes. The ideal candidate will have a strong background in nursing, excellent communication skills, and the ability to work independently in a fast-paced environment.

Responsibilities:
  • Provide high-quality, one-on-one care to patients in their homes
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Collaborate with the patient's healthcare team to ensure seamless care coordination
  • Monitor patient progress and adjust care plans as needed
  • Provide education and support to patients and their families on disease management and self-care
Requirements:
  • Current IL Registered Nurse (RN) license
  • 1 year of clinical experience preferred for field nurses
  • Current CPR certification (AHA Healthcare Provider)
