As a skilled nursing care coordinator with Traditions Health, you will play a vital role in administering skilled nursing care to clients of all ages in their place of residence. You will coordinate care with the interdisciplinary team, patients, and their families, as well as a referring agency. Your primary functions will include administering skilled nursing care, coordinating care, and assuming responsibility for the coordination of care.

Key qualifications for this role include a graduate degree from an accredited Diploma, Associate, or Baccalaureate School of Nursing, a current State license as a Registered Nurse, and a current Driver's License. Additionally, you will need one year of experience as a Registered Nurse in a clinical care setting, with home health experience preferred.

As a skilled nursing care coordinator, you will be responsible for completing initial and ongoing assessments to identify the physical, psychosocial, and environmental needs of home health patients/clients. You will also be responsible for developing, initiating, and revising the plan of care as necessary to ensure quality and continuity of care.

Other essential functions of this role include furnishing those services requiring substantial and specialized nursing skill, counseling patients and their families in meeting nursing and related needs, and using infection control measures that protect both the staff and the patient.
