About the Role:

This is an exciting opportunity for a licensed NP to utilize their skills and experience in delivering evidence-based medical care. You will collaborate with healthcare professionals to create comprehensive care plans and ensure seamless patient transitions.

Key Responsibilities:
  • Patient Assessment and Care Planning: Conduct thorough medical evaluations and develop personalized care plans tailored to each patient's needs.
  • Medical Services Delivery: Provide routine check-ups, chronic disease management, and urgent care services as needed.
  • Collaboration and Communication: Work closely with caregivers, family members, and other healthcare professionals to ensure effective communication and coordination of care.
Requirements:
  • Licensure and Certification: Active state Nurse Practitioners license and current NP National Board Certification.
  • Education and Experience: Master's degree in Nursing (MSN) and at least five years of nursing experience, including two years of nurse practitioner experience.
  • Skills and Abilities: Excellent interpersonal communication skills, strong knowledge of HIPAA and OSHA regulations, and proficiency in electronic medical record (EMR) systems.
