Job Summary:

We are seeking a skilled Senior Medical Professional to join our team at Abode Care Partners. This role involves providing high-quality medical care to seniors and patients with complex needs, ensuring their comfort and well-being.

About the Role:
  • Conduct evidence-based medical care, managing patient records and collaborating with healthcare professionals to enhance patient outcomes.
  • Work in various settings, including skilled nursing facilities, assisted living, independent living, group homes, and private homes.
Responsibilities:
  1. Medical Care Planning: Develop and implement comprehensive plans of care, taking into account patients' unique needs and circumstances.
  2. Patient Examination and Treatment: Conduct thorough examinations, diagnose conditions, and provide appropriate treatments, including medication management and referrals to specialists when necessary.
  3. Collaboration and Communication: Work closely with caregivers, family members, and other healthcare professionals to ensure seamless care coordination and effective communication.
  4. Documentation and Record-Keeping: Maintain accurate and up-to-date patient records, adhering to relevant laws and regulations.
Requirements:
  • Qualifications: Active state Nurse Practitioner license, current National Board Certification, active state DEA licensure, Master's degree in Nursing (MSN), and five or more years of nursing experience, including at least two years as a nurse practitioner.
  • Skills: Excellent interpersonal and communication skills, strong time management abilities, proficiency in medical instruments and equipment, and knowledge of HIPAA and OSHA regulations.

$100,000 - $130,000 per year, depending on experience and performance.
