We are seeking a qualified and caring Licensed Practical Nurse (LPN) to work in a group home setting in Branchburg, NJ.

This LPN will be responsible for assisting our developmentally disabled individuals with their daily needs, including administering medication, personal hygiene, and proper nutrition.

The ideal candidate will have experience working with individuals with intellectual and/or developmental disabilities and/or mental illness.

The LPN will work closely with our team to provide high-quality care and support to our residents.

The successful candidate will have a valid New Jersey LPN license, a valid New Jersey driver's license, and auto insurance.

They will also have clearance through Central Registry of Offenders Against Individuals with Developmental Disabilities, N.J.A.C. 10:44D.

Responsibilities include:

  • Recording temperature, weight, and blood pressure
  • Completing specialized individual care and treatment, such as tube feedings, care of the feeding site, tracheostomy care, and injections
  • Providing assistance required by individuals to ensure adequate standards of health care, administer medication, personal hygiene, proper nutrition, overall care, and participation of preferred activities
  • Assisting individuals with doctor appointments and community activities as permitted
  • Charting residents' care, treatment measures, including documentation through an internal system
  • Functioning as a liaison and advocate for the individuals served when dealing with physicians
  • Consulting with pharmacists and following up as needed
  • Completing monthly paperwork of individuals served health care needs
  • Monitoring the medical files of individuals served to ensure that all documentation is present and correct
  • Attending mandatory Nurses' Meetings as assigned
  • Complying with all standards to assure the health and safety of individuals served

The LPN will also be responsible for ordering medical supplies on a monthly basis for individuals served.
