Job DescriptionRole Summary:
As a Home Health Risk Assessment Nurse Practitioner, you will conduct comprehensive assessments of patients in their homes, identifying health risks and developing personalized care plans to improve health outcomes.
Key Responsibilities:
- Conduct thorough assessments of patients' medical history, medication adherence, social environment, and other risk factors
- Develop and implement personalized care plans to address identified health risks
- Collaborate with healthcare teams to ensure seamless care coordination
- Provide education and support to patients and their families to promote healthy behaviors and disease management
Requirements:
- Masters degree in Nursing or equivalent experience
- Current RN and NP licensure, including prescription authority
- Board certification in a relevant specialty
- Minimum 1 year experience as a Nurse Practitioner
- Strong computer skills and familiarity with electronic health records
