About the Role
This home health Occupational Therapist will provide comprehensive skilled services as ordered by the attending physician. The successful candidate will educate patients and their family members, ensuring patient safety, and remain compliant with all legal rules and regulations.

Key Responsibilities:
  • Plan of Care: Provide skilled occupational therapy as ordered by the attending physician.
  • Evaluation: Evaluate the patient's level of functioning by applying diagnostic and prognostic procedures.
  • Patient Outcomes: Use activities and home exercise programs to improve range of motion, muscle strength, and coordination through passive/active exercise and/or activity.
  • Minimize Patient Risk: Helps decrease re-hospitalizations by teaching the patient on preventative measures and making good decisions.

Requirements
To be considered for this role, you must have a current license in the state of employment, minimum one-year experience as a OT in an acute care setting, and valid driver's license and auto insurance in your name as a driver. Capable of all physical demands, compassionate communicator with a positive attitude, patience, attention to detail, and being observant and following directions are essential skills required for this position.
