Job Responsibilities
  • Plan and deliver comprehensive nursing care to patients in their homes, adhering to organizational philosophy, policy, and standards of practice.
  • Develop and implement individualized care plans that meet patients' needs, safety, and diagnosis for optimal outcomes.
  • Teach patients and families on an ongoing basis, providing education tailored to their learning style and needs.
  • Initiate and evaluate discharge planning, collaborating with the multidisciplinary clinical care team and accessing community resources as needed.
Requirements
  • Graduate of an approved School of Nursing
  • Current Massachusetts license as Registered Nurse
  • Minimum of one-year recent nursing experience in acute or emergency setting required
  • Home Care experience a plus
