We are seeking a skilled Hospice Case Manager to join our team at Abode Hospice and Home Health. As a key member of our interdisciplinary team, you will play a vital role in providing exceptional care to our patients and their families.
About the RoleIn this role, you will have the opportunity to make a meaningful impact on the lives of others by providing direct care and coordinating with the healthcare team to ensure that our patients receive the best possible care. Your expertise and compassion will be essential in helping us deliver our mission of bringing first-class care to the warmest and most serene environment possible – home.
Responsibilities- Conduct initial and ongoing comprehensive assessments of patients and families to identify their unique needs and develop personalized care plans.
- Collaborate with the healthcare team to initiate and reevaluate patient plans as necessary, ensuring that our patients receive timely and effective care.
- Utilize health assessment data to determine nursing diagnoses and administer medication and treatment as prescribed by the physician in the plan of care.
- Counsel patients and families in meeting nursing and related needs, promoting education and empowerment throughout the care journey.
- Document accurate clinical notes and communicate effectively with the healthcare team to ensure seamless coordination of care.
- Graduate of an accredited nursing school with current licensure to practice in the state of operation.
- At least 1 year of nursing experience, with 3+ years preferred.
- Experience in hospice or a similar setting is highly desirable, but not required.
- Demonstrated knowledge of the hospice philosophy of care and commitment to clinical and documentation excellence.
