About the Role

Hartford Healthcare is seeking a Full-time Nurse Practitioner or Physician Assistant to join our REACH Program serving geriatric patients in the greater Hartford area.

Job Overview

This is an exciting opportunity for a dedicated healthcare professional to work as part of a multidisciplinary care team providing high-quality patient care in a home setting. The ideal candidate will possess excellent clinical skills, strong communication abilities, and a passion for delivering compassionate care to older adults.

Responsibilities

  • Perform routine physical examinations and formulate medical plans to treat patients
  • Develop plans for further evaluation of health needs in consultation with the physician(s) and care team members
  • Provide appropriate treatment for problems within the scope of practice as designated by protocols and identify problems outside of their scope of practice and refer appropriately to physicians

Requirements

To be successful in this role, you will need:

  • An unrestricted State of Connecticut Medical License (or eligible)
  • APRN (Family, Adult/Geri Nurse Practitioner) or PA training
  • At least two years of post-graduate experience as a Nurse Practitioners (APRN) or Physician Assistants (PA)

Earnings Potential

We offer a competitive salary range of $110,000 - $140,000 per year, based on experience and qualifications. This amount is subject to change and may not reflect additional forms of compensation, such as bonuses or overtime pay, which may be applicable to certain employees.
