Job Summary:

We are seeking a highly motivated Communications Specialist to join our team. As a key member of our interdisciplinary team, you will be responsible for evaluating and assessing speech, language, cognitive-communication, and swallowing disorders in residents of skilled nursing facilities (SNF). You will develop and implement individualized treatment plans to address communication and swallowing deficits, enhancing residents' quality of life.

Key Responsibilities:

  • Evaluate and assess speech, language, cognitive-communication, and swallowing disorders in SNF residents
  • Develop and implement individualized treatment plans to address communication and swallowing deficits
  • Provide direct therapy services, including individual and group sessions, to improve speech, language, and swallowing functions
  • Educate residents, families, and caregivers on strategies and techniques to support communication and swallowing needs at home and in the facility
  • Collaborate with interdisciplinary teams, including physicians, nurses, occupational therapists, and physical therapists, to ensure comprehensive care for residents
  • Document evaluations, treatment plans, progress notes, and discharge summaries in compliance with healthcare regulations and facility policies
  • Participate in care planning meetings, providing expert input on residents' communication and swallowing needs

Requirements:

  • Master's degree in Speech-Language Pathology from an accredited program
  • Current state licensure or certification as a Speech-Language Pathologist (SLP)
  • Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) preferred
  • A minimum of 1-2 years of experience in a skilled nursing facility or geriatric setting
  • Strong knowledge of speech, language, and swallowing disorders, particularly in the elderly population
  • Excellent communication and interpersonal skills
  • Ability to work effectively as part of an interdisciplinary team
