Resident Care Manager RN

Avamere Communities is seeking a dedicated and compassionate Resident Care Manager RN to join our team at Avamere Rehab of Beaverton. As a key member of our nursing staff, you will be responsible for providing exceptional care to our residents and ensuring their needs are met.

Responsibilities:
  • Develop and implement individualized care plans for residents, ensuring their medical and nursing needs are met.
  • Supervise and direct nursing staff, providing guidance and support to ensure high-quality care.
  • Conduct daily rounds to monitor resident care and address any concerns or issues.
  • Collaborate with the Director of Nursing Services to ensure seamless care delivery and address any concerns or issues.
  • Participate in facility surveys and make recommendations for new or changes in procedures, policies, methods, education, reference material, and general nursing practices.
Qualifications:
  • Current, unencumbered, active license to practice as an RN in this state.
  • Minimum 1 year of experience as a supervisor in a healthcare setting.
  • Training in rehabilitative and restorative nursing practices.
  • Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities.
  • Experience with Electronic Medical Records and computer documentation systems.
