Mass General Brigham is seeking a skilled Registered Nurse to work as a Senior Heart Disease Care Coordinator in Boston, Massachusetts. This role involves coordinating the care of patients with advanced heart disease and ensuring that they receive high-quality treatment.

About the Role

This position requires strong clinical knowledge and experience in heart failure management. The successful candidate will be responsible for organizing cardiac and non-cardiac testing, collecting and reporting results, and sharing information with internal and external providers as needed.

The Senior Heart Disease Care Coordinator will work closely with multidisciplinary teams, including physicians, nurses, social workers, pharmacists, physical and occupational therapists, and care coordinators to ensure that patients receive comprehensive care. They will also communicate with community agencies and maintain ongoing care and support for patients.

Key Responsibilities
  • Clinical Activities: Coordinating evaluation of advanced heart failure patients being considered for mechanical circulatory support, including left and/or right ventricular assist device (VAD). Responsible for organization of cardiac and non-cardiac testing, collecting and reporting results, and entering clinical information into electronic databases.
  • Care Coordination: Collaborates with unit staff, MCS nurse practitioners, and physicians to facilitate a comprehensive plan of care and coordinate resources.
  • Patient Education: Provides education to MCS patients and family members as well as inpatient staff nurses and house staff regarding important aspects of care, including but not limited to device function and maintenance, activity level, diet, medication adherence and healthcare follow-up.
  • Regulatory Compliance: Assists in maintaining a state of continuous regulatory readiness and collaborates with MCS NPs, physicians and nursing leadership, as well as hospital quality and compliance personnel to ensure MCS regulatory compliance as defined by Joint Commission standards.
Requirements
  • Graduate of an approved school of nursing with current registration in Massachusetts.
  • Bachelors of Science in Nursing - BSN required.
  • Minimum 3 years of experience with cardiology/MCS-focused patient care experience required.
  • Triage experience or telephone-based clinical assessment technique required.

The estimated salary for this role is $93,000 - $118,000 per year, depending on experience.
