We're seeking a skilled HCBS Wait List Care Coordinator to join our team in Reno, Butler, and Sedgwick counties and surrounding areas. As a member of our team, you'll have the opportunity to make a meaningful impact on the lives of our customers.
The estimated salary for this position is $62,000 - $82,000 per year, depending on experience. This is a field-based position with a home-based office, requiring local travel up to 75% of the time. You'll work from home when not in the field, with reimbursement for mileage at the current government rate.
About the JobThis role requires a Bachelor's Degree in social work, rehabilitation, nursing, psychology, special education, gerontology, or a related human services area. You'll need at least one year of experience working with persons with long-term care needs and within the community health setting in a healthcare role.
Your primary responsibilities will include assessing, planning, and implementing care strategies tailored to individual needs. You'll identify and initiate referrals for social service programs, communicate with stakeholders, and advocate for individuals as needed.
You'll also conduct home visits, serve as a resource for community care coordinators, and maintain good judgment and accountability while working independently. Strong communication, customer service, and organizational skills are essential for success in this role.
