Key Responsibilities:
  • Provide comprehensive nursing care to patients in their homes under the direction of a registered nurse.
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team.
  • Provide health education to patients and caregivers, empowering them to take control of their health.
  • Communicate effectively with physicians, patients, and families to ensure seamless care coordination.
  • Document patient care accurately and timely, ensuring compliance with regulatory requirements.
Requirements:
  • Active LPN license in the state of Connecticut or compact license.
  • Minimum 6 months LPN pediatric experience (nice to have).
  • LPN home health experience preferred.
  • Reliable transportation to perform job duties.
