Community Case Manager

Group Main Stream is seeking a skilled Community Case Manager to join our team. As a Community Case Manager, you will be responsible for providing person-centered services to individuals with diverse needs. Your primary goal will be to facilitate effective relationships with individuals, their families, and provider agencies to promote empowerment, community inclusion, and health and safety.
Key Responsibilities- Consumer Support
- Maintain a caseload of 18-25 individuals, adhering to a 35:1 ratio as outlined in the Case Management manual.
- Develop and maintain relationships with individuals, their families, and other agencies to ensure necessary observation and assessment of needs.
- Maintain the rights of individuals and advocate for their needs.
- Ensure case management activities are directly linked to a person-driven needs-based model.
- Create and monitor Person-Centered Plans with adherence to accepted treatment principles and techniques.
- Safety
- Ensure the implementation of plans with a focus on the health and safety of individuals through Group Main Stream's systems.
- Promote safe work practices among all employees.
- Documentation
- Complete all documentation as required to provide high-quality services, complete billing, and maintain waiver classification.
- Document all communication, screening summaries, evaluations, and observations as they occur.
- Leadership
- Attend conferences, workshops, and seminars to improve professional skills and benefit the Agency, individuals served, and staff.
- Represent Group Main Stream in the community and serve as a member on relevant committees and work groups.
- Education
- Bachelor's degree from an accredited four-year institution of higher learning with a specialization in psychology, mental health, and human services, behavioral health, behavioral sciences, social work, human development, special education, counseling, rehabilitation, sociology, nursing, or a closely related field.
- Experience
- At least one year of full-time equivalent relevant human services experience.
- Skills
- Possess professional values and ethics.
- Model confidentiality practices as stated in federal, state, and Group Main Stream policies.
- Possess basic computer skills and a working knowledge of Microsoft Word programs.
- Possess a valid Maine state driver's license.
- Possess good verbal and written communication skills.
- Ability to obtain and maintain all required certifications and trainings.
- Ability to pass required background checks.
$24 hourly
