Job Summary

Clinical Liaison Representatives at Spaulding Rehabilitation Hospital play a vital role in ensuring a smooth transition of patients to post-acute services within Mass General Brigham's scope of care. As a key member of our team, you will participate in referral relations activities, conduct preadmission patient evaluations, and coordinate the referral process to ensure seamless transitions.

Key Responsibilities

  • Evaluate patients, interview them, and refer them to appropriate post-acute services within Mass General Brigham's scope of care, adhering to established guidelines and local, state, and federal regulations.
  • Perform on-site clinical judgments to determine patient appropriateness for admission.
  • Obtain patient assessment information from various sources and transmit data to appropriate systems.
  • Develop and maintain collaborative relationships with internal staff and external referral sources to ensure customer satisfaction.
  • Nurture referral source relationships to maintain customer satisfaction and support challenging transfers using Mass General Brigham's clinical and supervisory resources.
  • Communicate pertinent information from Mass General Brigham to current medical providers to meet patient and provider needs.
  • Identify and communicate Mass General Brigham referral influences to the manager for proper follow-up.
  • Consider discharge plan alternatives to determine the appropriate plan to meet patients' needs.
  • Achieve payor mix results and account/territory referral and admissions targets.
  • Share educational information and updates with referral sources to ensure an understanding of Mass General Brigham's post-acute services.
  • Complete additional department, organization, or network activities per established objectives.
  • Uphold Spaulding Organizational Values of Innovation, Collaboration, Accountability, Respect, and Excellence.

Qualifications and Experience

Required:

Graduate of an accredited RN, PT, OT, or SLP program.

Minimum 2 years of acute hospital or extended care facility experience.

Current license in Massachusetts as RN, Physical Therapist, Occupational Therapist, or Speech Language Pathologist.

Knowledge of switchboard practices, paging procedures, and equipment.

Computer proficiency, with Microsoft Office applications preferred.

Effective verbal, electronic, and written communications skills using the English language.

Preferred:

Broad knowledge of various insurances and government reimbursements.

Sales and/or marketing experience.

Highly developed communications and interpersonal skills, working with diverse populations.

Ability to work independently, be self-directed, and contribute as a member of a team.

Anticipates challenges and develops strategies for addressing them.

High level of service delivery, with ability to prioritize work, meet deadlines, and adapt to changing situations.

Attention to detail.
