We are seeking a dedicated Developmental Program Coordinator to join our team at CDS Monarch, Inc. in Webster and Penfield Day Programs.

Job Summary

This is a full-time position that requires the ability to work independently and motivate others. The successful candidate will provide supervision and direction to all staff and participants in their assigned module or group, ensuring implementation of the participant's IPP, integration of all services, and progress made by the participant.

Responsibilities
  • Program Coordination: Coordinate with members of the Team, participants' families, and residences to facilitate program planning and maximize optimum functioning of the participant in his/her program.
  • Habilitation Skills Assessment: Assess habilitation skills of participants assigned to the group and develop, implement, and document outcomes for all assigned participants.
  • Documentation: Ensure documentation is completed for services provided, including data sheets, outcome performance records, progress notes, travel logs, etc.
  • Training and Supervision: Provide training to staff as needed and supervise the module's assigned staff, providing ongoing continuous supervision throughout the program day.
Requirements

To be successful in this role, you will need:

  • A Bachelor's degree preferred and meets QMRP status per 690 regulations;
  • Bachelor's degree in a field of education, social work, psychology, rehabilitation, or related human services field from an accredited program;
  • Holds permanent certification from the New York State Education Department in a field of education;
  • Specialized training or one year of experience in treating or working with the developmentally disabled;
  • Associate's degree + 3 years experience; or High School diploma plus 4 years experience in treating or working with developmentally disabled individuals.
