About the Role

We are seeking a skilled Care Coordinator to join our team at Argus Home Healthcare. As a Care Coordinator, you will be responsible for providing high-quality nursing care to patients in their homes, ensuring their comfort and well-being.

Key Responsibilities:

  • Patient Assessment: Conduct comprehensive health assessments of patients to identify their needs and develop personalized care plans.
  • Care Planning: Collaborate with patients, families, and healthcare teams to create and implement effective care plans that address physical, emotional, and social needs.
  • Medication Management: Administer medications and treatments as prescribed by physicians, and provide patients with instructions on proper medication use.
  • Educational Support: Teach patients and their families about self-care techniques, healthy habits, and disease management strategies.
  • Communication: Maintain open communication with patients, families, and healthcare providers to ensure seamless care transitions and addresses any concerns or issues.

Requirements:

  • Nursing Degree: Possess an Associate's or Bachelor's degree in Nursing (RN) from an accredited institution.
  • Licensure: Hold a current RN license in the state where services will be provided, along with CPR certification and a valid driver's license.
  • Experience: Have at least one year of experience working as a professional nurse in an acute care setting, preferably in home health care.
