The Care Manager is the most senior level of direct care staff working within our supportive housing programs to help residents live in the community as independently as possible. Key responsibilities include carrying a caseload, practicing person-centered and collaborative service planning with residents, and ensuring service plans are completed and signed by residents according to regulatory guidelines and standards.

Additional responsibilities include evaluating and assessing independent living skills, supporting residents in meeting individual goals, and advocating on behalf of residents with other services providers. You'll also participate in monthly collaborative walk-throughs of unit/apartments with residents, escort residents to appointments as needed, and assist residents with physically demanding tasks.

This position requires a Bachelor of Arts (BA) or Bachelor of Science (BS) degree, and at least 1 year of experience in Human Services. You'll also need to be familiar with Microsoft Office suite of products and have the ability and willingness to learn new systems and application software.

**Job Requirements:**
