Hospice Care Specialist

HopeHealth Hospice and Palliative Care is seeking an experienced Certified Nursing Assistant to join its dynamic and caring hospice team. This position will provide care to hospice residents in long-term care and assisted living facilities in the northern half of Rhode Island.
The role of a hospice aide is to provide personal care and light housekeeping duties in accordance with the established plan of care, as assigned by a registered nurse. This is a full-time, 40-hour-a-week position with 11:00am-7:30pm hours, offering a competitive compensation package, health care coverage, education reimbursement, and an employer-match retirement plan.
Duties and Responsibilities:
- Provide personal care to patients, including bathing, dressing, and grooming
- Observe, report, and document patient status and the care provided
- Communicate effectively with patients, families, and caregivers to provide support and address concerns
- Follow basic infection control procedures to ensure a safe environment
- Recognize and report changes in patient status to the RN Case Manager
- Respond to emergencies and apply knowledge of emergency procedures
- Maintain patient and family confidentiality
Requirements:
- Certified Nurse Aide certification
- High School Diploma or GED
- 1-3 years of experience in a healthcare setting
