Position Overview
We are seeking an experienced Occupational Therapist to provide therapy interventions and treatment in the comfort of patients' homes in Eau Claire and surrounding cities.
About the Role
This is a full-time position with a salary range of $85,000-$110,000 per year, depending on experience. Our Occupational Therapist will work Monday through Friday, delivering occupational therapy services to comply with prescribed physician orders.
Key Responsibilities
- Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and current level of function.
- Administer, evaluate, and interpret tests and measurements to determine functional goals and outcomes.
- Develop and implement individualized plans of care defining occupational therapy needs, goals, progress, and expected outcomes.
- Maintain accurate and timely documentation of client performance, modifying goals and treatment as needed, and determining discharge care plans as appropriate.
- Collaborate with healthcare team members, including physicians, nurses, and other therapists, to ensure comprehensive care.
- Supervise and train other personnel as applicable.
- Complete OASIS assessments where allowed by state professional practice and regulation.
Requirements
- Degree in Occupational Therapy from an accredited university/program.
- Current, unrestricted state license as an Occupational Therapist in the state of practice.
- Valid CPR certification.
- Strong organizational and communication skills, attention to detail, time management, and ability to build strong relationships with patients, families, and healthcare teams.
- Prior experience in Home Health Therapy and completion of OASIS documentation are preferred.
