About the Role:

Cantex Continuing Care Network is seeking a skilled Physical Therapy Assistant to join our team in providing high-quality home health care services.

Key Responsibilities:

  • Provide coordinated physical therapy to patients of all age groups under the supervision of a licensed Physical Therapist.
  • Collaborate with the clinical team and patient to promote patients' potential during their home health episode.
  • Record treatments administered in medical records and maintain accurate clinical documentation.
  • Communicate patient status updates to supervising PT and participate in in-service training programs.

Requirements:

  • Physical Therapy Assistant license per state requirements.
  • ~2 years of Home Health experience preferred.
  • Ability to read, write, and speak English fluently.
