Job Summary

We are seeking a skilled Patient Care Coordinator to join our team at Abrazo West Campus. As a key member of our psychiatric care team, you will be responsible for providing high-quality care to adult, adolescent, and child patients. Your primary focus will be on planning and implementing milieu treatment, considering patient needs and therapeutic goals. You will work closely with our multidisciplinary team to ensure the safe and effective delivery of care to patients of all ages.

Key Responsibilities

1. Collaborate with the nurse therapist to determine a milieu treatment approach and provide consistent individual counseling focused on treatment goals during hospitalization. Participate in milieu therapy for patients on an individual or group basis, leading or assisting in milieu groups as needed. Incorporate knowledge of each patient's condition and apply various therapeutic techniques to ensure appropriate participation by patients.

2. Work with other unit staff to assess and evaluate patient mental status, initiate and maintain a therapeutic relationship with patients, and collaborate in the diagnosis and treatment of patients. Share information and participate in the formulation of treatment plans, coordinating and implementing various aspects of the milieu environment to provide a therapeutic experience for patients. Collaborate with other staff members to determine whether a patient represents a risk to themselves and/or others, working with other unit personnel to develop crisis intervention techniques.

3. Provide supportive care to patients, including participating in conversation, recreational activities, and basic living activities. Assist patients with bathing, eating, and other daily living activities as needed, setting limits, confronting counterproductive activities and behavior, and reassuring and orienting patients. Encourage and facilitate appropriate interactions between patients.

4. Record all appropriate information on patient charts, preparing regular reports describing each patient's mental status and functioning, as well as significant interactions. Report and document overt, covert, verbal, and non-verbal communications by patients.

5. Provide support and education to family members and significant others regarding the patient's status, working with representatives of outside agencies to obtain or disseminate information on individual patients. Participate in discharge planning and referral, if necessary.

Requirements

• Bachelor's Degree in psychology or a related field preferred
• Bilingual preferred (Spanish/English)
• BLS required

Working Conditions

1. Patient care environment, with periodic risk from combative or abusive patients; occasionally may assist with moving or lifting elderly or handicapped patients or with applying restraints.
2. Regular walking and standing during normal course of duties.
