Medical Care Coordinator for Youth in Residential Treatment

At OhioGuidestone, we are dedicated to providing high-quality behavioral health services to individuals and families in need. We are currently seeking a compassionate and skilled Medical Care Coordinator to join our team at our Children's Residential Treatment Facility in Berea.
About the Role
The successful candidate will be responsible for coordinating medical care for youth residing at our facility, working closely with our medical providers, therapists, and support staff to ensure that each resident receives comprehensive and individualized care. This role requires strong organizational and communication skills, as well as the ability to work effectively in a fast-paced environment.
Key Responsibilities
- Coordination of Medical Care: Develop and implement individualized care plans for each resident, including coordination of medical appointments, medication management, and monitoring of vital signs.
- Communication and Collaboration: Maintain open lines of communication with medical providers, therapists, and support staff to ensure seamless delivery of care.
- Documentation and Record-Keeping: Accurately document all aspects of medical care, including progress notes, medication records, and laboratory results.
- Client Engagement and Support: Provide emotional support and guidance to residents and their families, promoting a sense of security and stability.
Requirements
To be successful in this role, you will need:
- Nursing License: A valid RN or LPN license in the state of Ohio is required.
- Experience: Previous experience in a medical setting, preferably in a residential treatment facility or similar environment.
- Skills: Excellent communication and organizational skills, with the ability to work effectively in a fast-paced environment.
