About the Role

As a skilled Home Health Clinician, you will play a vital role in delivering high-quality patient care in the comfort of their homes. This position offers a unique opportunity to work with a diverse range of patients, providing comprehensive assessments and developing personalized care plans to address their specific needs.

Key Responsibilities
  • Conduct thorough home visits to assess patients' physical and mental health
  • Develop and implement individualized care plans to address identified needs and goals
  • Collaborate with healthcare teams to ensure seamless care coordination and effective communication
  • Provide education and support to patients and their families on disease management, medication adherence, and healthy lifestyle choices
Requirements
  • Active NP or PA license in Washington State
  • National certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification (for NPs) or Physician Assistant national certification through NCCPA (for PAs)
  • Ability to complete physical requirements of the job, including lifting 30 pounds and navigating various dwelling conditions
Preferred Qualifications
  • 1+ years of clinical experience in gerontology or a related field
  • Experience working with patients with complex medical conditions or those requiring specialized care
  • Fluency in Spanish, Cantonese, Korean, Vietnamese, Polish, or other languages (dependent on patient population)
Compensation and Benefits

The estimated salary for this role is $88,000 to $173,200 annually, based on local labor markets, education, work experience, certifications, and other factors. UnitedHealth Group offers a comprehensive benefits package, including:
