Eventus WholeHealth is seeking a skilled Licensed Clinical Social Worker to join our multidisciplinary team in providing high-quality, patient-centered care to residents of skilled nursing and assisted living facilities.
About the RoleThis is an exciting opportunity for a mental health professional to make a meaningful difference in the lives of vulnerable adults. As a Licensed Clinical Social Worker, you will assess each resident's needs, develop individualized treatment plans, and provide recovery-oriented rehabilitation services in group or individual settings.
You will be working collaboratively with a team of healthcare professionals, including physicians, nurse practitioners, and support staff, to deliver comprehensive care that addresses a wide range of problems, from anxiety and depression to severe and persistent mental illness.

Key Responsibilities- Assessment and Treatment Planning: Assess each resident's needs, develop and follow individualized treatment plans, and ensure that all treatment provided is medically necessary.
- Collaboration and Communication: Perform written or verbal consultation with clients' primary care physician, family, facility staff, and Eventus WholeHealth support staff as needed.
- Documentation and Reporting: Document treatment plans, psychotherapy sessions, and treatment plan updates according to Eventus WholeHealth's procedures and policies and provide the facility with written documentation within 48 hours of service for inclusion in the patient's facility medical record.
- Professional Development: Establish and maintain open and positive communications with facility staff and administration. Provide verbal and/or written instruction or feedback regarding treatment plans and other pertinent caregiver information.
To be successful in this role, you will need:
- A Master's degree in social work and a current license to practice as a Licensed Clinical Social Worker in the state where you will be practicing.
- Excellent assessment and treatment planning skills, with experience in providing recovery-oriented rehabilitation services in group or individual settings.
- Strong communication and collaboration skills, with the ability to work effectively with healthcare professionals, families, and facility staff.
- A commitment to delivering high-quality care and making a positive impact on people's lives.

We are a dynamic and growing organization, and we offer competitive salaries and benefits to our employees. Our salary for this role is $ $90,000 - $120,000 per year, depending on experience and qualifications.
