About the Role:

We are seeking a skilled and compassionate Home Health Physical Therapist to join our team in Eagle Lake, Texas. This is a full-time opportunity with a competitive salary range of $90,000-$105,000 per year.

Responsibilities:
  • Provide high-quality physical therapy services to patients in their homes
  • Develop and implement personalized treatment plans to achieve patient goals
  • Collaborate with interdisciplinary teams to ensure seamless care coordination
Requirements:
  • Master's degree in physical therapy from an accredited program
  • Current licensure as a physical therapist in the state of Texas
  • At least 9 months of experience as a physical therapist, preferably in home health or community settings
