About the Role: Primary Care Provider - In Home Care

As a Primary Care Provider, you'll have the opportunity to work closely with patients in their own homes, providing personalized care and education that promotes healthy living and disease prevention. Your expertise will help us achieve our goal of improving health outcomes and reducing healthcare costs.

Key Responsibilities:
  1. Conduct comprehensive assessments, including Annual Wellness & Quality Gap Care visits, to identify patients' needs and develop effective care plans.
  2. Maintain accurate and up-to-date records of patient interactions, clinical findings, and treatment plans.
  3. Collaborate with our interdisciplinary team to develop and implement care plans that address patients' medical and social needs.
  4. Stay up-to-date with industry best practices, regulations, and guidelines to ensure high-quality care and compliance.
Requirements:
  • At least 2 years of experience as a Primary Care Provider.
  • Certification in a relevant specialty, such as geriatric or adult medicine.
  • Familiarity with HCC models, HEDIS, and STAR measures.
  • Able to work independently and collaboratively as part of a team.
Compensation and Benefits:

We offer a competitive salary of $120 per completed visit, payment for same-day no-show visits, independent contractor status, reimbursement for travel mileage, and access to ongoing training and professional development opportunities.
