About the Role

We are seeking a skilled Home Health Nurse Practitioner to provide in-home health and wellness assessments for various health insurance plans. This opportunity is ideal for a dedicated professional with experience in geriatric care.

Key Responsibilities
  • Conduct comprehensive H&P evaluations, including reviewing medications and dosages.
  • Perform physical examinations, check vital signs (blood pressure, finger stick), and collect urine samples.
  • Leave fecal kits for patients to mail in.
  • Complete approximately 5-8 assessments per day, lasting around 45-75 minutes each.
Requirements
  • MUST HAVE CAQH, NPI #, NP license in the state of work, and an unrestricted state license.
Benefits
