Job Summary

This Clinical Social Worker Specialist will work in collaboration with the healthcare team to provide psychosocial services for patients with end stage renal disease. The successful candidate will promote positive adjustment, rehabilitation, and improved quality of life for our patients.

Responsibilities:
  • Assess patient psychosocial status, strengths, and areas of need that may affect rehabilitation and optimal treatment outcomes as part of comprehensive patient assessment.
  • Collaborate with the patient and healthcare team to identify effective interventions that will help the patient meet rehabilitation, treatment goals, and improve quality of life.
  • Utilize patient education programs, established social work theory and methods, social work focused interventions, and quality of life measurement instruments as part of the assessment and care planning to address barriers and meet patient treatment goals.
Requirements:

Masters in Social Work; State required license; 0-2 years' related experience.

Compensation:

The estimated salary for this position is around $70,000-$90,000 per year, depending on location and experience.
