Job Summary

We are seeking a dedicated Hospice Social Worker to provide compassionate care and support to patients and their families in a healthcare setting. The ideal candidate will possess a Bachelor's Degree in Social Work and relevant licensure, with a minimum of one year of experience in a healthcare or hospice environment.

Key Responsibilities
  • Provide psychosocial support to patients and their families, utilizing professional training and personal judgment.
  • Assess patient needs and develop individualized care plans to address their physical, emotional, and social needs.
  • Collaborate with interdisciplinary teams to ensure comprehensive care and support.
  • Document patient interactions and progress in accordance with regulatory requirements.
Requirements
  • Bachelor's Degree in Social Work from a CSWE-accredited school.
  • Licensure as required by the state in which the hospice is located.
  • One year of experience as a Social Worker in a healthcare and/or hospice setting.
