About the Role

The Case Manager will serve as a member of the Prevention team at WestCare, working to deliver quality services to individuals served and their families. This role provides case management to eligible veterans and their families in suicide prevention, awareness, and community resource linkage.

Key Responsibilities
  • Case Management: Support program alignment with the VA's National Strategy for Preventing Veteran Suicide (2018), and provide first or second initial contact with eligible veterans and/or family.
  • Service Coordination: Schedule appointments to complete program eligibility process, review required documentation, and develop and review recovery treatment plans.
  • Community Resource Linkage: Assist eligible veterans and their families with accessing employment, housing assistance, and other needed veteran services.
  • Documentation and Reporting: Maintain all case files, input data in tracking system, and submit weekly workplans/reports on activities.
Requirements
  • Education: Graduation from an accredited two-year Community College or a four-year College/University with a degree in Communication, Sociology, Criminal Justice, Social Work, Psychology, or related human services field.
  • Experience: Previous military employment preferred, with at least 3-5 years of experience in a veteran service-related field.
  • Certifications/Licenses: Valid driver's license required.
