We are seeking a compassionate and dedicated Hospice Aide / CNA to join our team at AccentCare, Inc.

Job Description:

Overview

As a Hospice Aide / CNA, you will play a vital role in providing high-quality patient care and support to individuals in need. You will work closely with our interdisciplinary team to deliver compassionate and personalized care that meets the unique needs of each patient.

Responsibilities:

  • Provide direct patient care, including bathing, dressing, and grooming
  • Assist patients with daily living activities, such as eating, toileting, and transferring
  • Monitor patients' vital signs and report any changes to the healthcare team
  • Assist with medication administration and documentation
  • Participate in patient and family education
  • Collaborate with the healthcare team to develop and implement individualized care plans

Requirements:

  • Licensed to practice as a certified nursing assistant in the state of Texas
  • High School Diploma or equivalent
  • Valid CPR certification
  • Licensed driver with a valid driver's license
