About the Role:

We are seeking a highly skilled and compassionate House Calls Medical Specialist to join our team at Abode Care Partners.

Overview:

As a leading provider of integrated medical services, we care for individuals from post hospitalization to home in various settings. Our comprehensive approach prioritizes quality medical care for seniors, people with complex conditions, special needs, and individuals with intellectual and/or developmental disabilities.

We foster an environment where healthcare professionals can grow their skills while delivering personalized care to patients in their homes and living facilities.

Responsibilities:
  • Conduct thorough patient assessments to identify medical needs and develop individualized care plans.
  • Manage chronic and urgent illnesses, minor injuries, accidents, and other medical issues effectively.
  • Order and interpret laboratory and diagnostic testing as necessary to ensure accurate diagnoses and treatments.
  • Collaborate with caregivers and other healthcare professionals to provide interdisciplinary care and maintain comprehensive patient records.
  • Provide referrals to other healthcare practitioners when required and stay updated on best practices in patient care.
Requirements:
  • An active state license for Nurse Practitioners (NP).
  • Current National Board Certification for Nurse Practitioners.
  • Active DEA licensure and a Master's degree in Nursing (MSN).
  • A minimum of five years' nursing experience, with at least two years as a Nurse Practitioner preferred.
  • Excellent communication and interpersonal skills, both verbal and written.
  • Strong time management and organizational skills to prioritize tasks efficiently.
  • Familiarity with medical instruments, equipment, HIPAA, OSHA regulations, and electronic medical record systems.
  • Ability to engage in moderate physical activity and drive safely.
