Job Description

We are looking for an experienced RN Care Manager who can work collaboratively with interdisciplinary teams to coordinate care for complex patients from pre-admission to immediate post-discharge. The ideal candidate will have a strong background in acute care hospital or community-based clinical nursing, with a minimum of 3 years of experience.

Responsibilities:

  • Coordinate care for complex patients to ensure seamless transitions between healthcare settings
  • Work collaboratively with inter-disciplinary teams to improve patient outcomes
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Monitor and evaluate patient progress, making adjustments to care plans as needed

Requirements:

  • Associate's Degree in Nursing
  • Washington Registered Nurse License
  • 3 years of Acute care hospital or community-based clinical nursing experience
